Despite substantial declines in gastric cancer incidence, the incidence of adenocarcinoma esophagogastricjunction (AEG) has been rising remarkably worldwide and is drawing considerable attention as well concern.However, optimal extent esophagogastric resection for this tumor entity, especially Siewert type II tumorslocated at true cardia, remains highly controversial. Given uncertainty about prophylactic lymph node dissection, both subtotal esophagectomy extended total gastrectomy have advocatedfor entity. A recent Japanese questionnaire-based national retrospective study showed that completenodal clearance along distal portion stomach offered marginal survival benefits patients with AEGless than 4 cm diameter. prospective nationwide multicenter accurately identified distribution oflymph metastases from advanced AEG, suggesting it not recommended to completely dissect lymphnode stations stomach. Total might be employed less frequently andconsequently replaced by or proximal clinical practice.
